| The
Christian Brothers Employee Benefit Trust offers
a comprehensive benefit package to any non-profit
Catholic organization approved for or listed in the
Kenedy Official Catholic Directory. This package
may include Life Insurance with Accidental Death & Dismemberment,
Long-Term Disability Income Benefits and Medical
Coverage with Prescription Drug Card benefits, Dental
Coverage (which may or may not include orthodontic
benefits) and Vision coverage.
Group Life Insurance
Provided to each eligible employee. Proceeds to
be paid to the beneficiary designated by the
employee. Includes Accidental Death and Dismemberment
benefits.
Life insurance is available in a multiple of salary:
1 times salary; 1 ½ times salary; 2 times
salary; to a maximum of $150,000. Also available
in flat amounts of $10,000/employee, $20,000 employee,
etc.
Group Long Term Disability (LTD)
Provided to each eligible employee. Pays a percentage
of salary to those disabled due to injury or
illness. LTD permits income benefits after a
90-day waiting period. A benefit of 70% is received
in coordination with other coverage, including
rehabilitation benefits.
Medical
The Medical Plan provides comprehensive benefits,
including preventive care, with a choice of deductibles
and co-payment levels.
Managed care has brought about cost-effective plan
utilization without sacrificing quality. The Trust
has negotiated with several regional PPO
networks to obtain significant discounts off
billed charges.
Prescription Drug Card Programs are available
with all medical plan designs. The Integrated Plan
provided through Medco Health offers prescription
service at both retail and mail order pharmacies.
The program
includes over 99% of all retail pharmacies across
the United States. Our long-term mail order program
can offer significant savings to participants
with the ease of no hassle refills.
Vision
Although the medical plan provides coverage for
medical conditions, our vision plan assists
in covering the cost for exams, lenses
and frames.
The participants
are
able
to seek care from any provider they choose.
Dental
Our dental plan is a comprehensive plan providing
preventative, basic, and major dental care, with
the freedom of choice of providers.
We also have available
an Orthodontia plan that assists in the cost
of orthodontia care.
